Euclid Telehealth Partner | Cor Medical
Eye disease screening for early detection and treatment of vision loss is offered by Euclid Telehealth with your family physician at your Cor Medical Centre at McLeod.
We are happy to let you know that Euclid Telehealth and your doctor at Cor Medical Centre are working together to bring you a specialized eye disease screening covered by Alberta Health Care, for the early detection, prevention, and treatment of vision loss. Euclid will be on-site at our clinic, every Thursday of the month, to offer a non-invasive 20-minute eye disease screening. Based on your doctor’s recommendation, Euclid will be contacting you to schedule an appointment, but in the meantime, you can learn more or book online at https://euclidtelehealth.org/cormed, or contact Euclid’s team at patientsupport@euclidtelehealth.org, or call 1-800-511-5661 to speak with their patient support team who will be happy to answer any questions you may have.

We have now implemented an online appointment booking service for Family Practice patients only. If your family doctor is here at our clinic, then please go to the following link or scan the QR code to book an appointment.
● The online booking is STRICTLY for non emergency booking. If this is a life threatening concern please go to the nearest emergency/call 911. If you need advice from a nurse please dial 811 Health link.
● The online booking is STRICTLY for a simple consult such us refills, follow up, regular consult. Please give our clinic a call if booking for consulting relating to work related injury for the first time, any forms/letters/notes, or for complete physical check up.
● Appointments can still be made with our reception by calling (403) 475-0369, or going to our website cormed.ca

HOW TO FIND US
Cor Medical Centre is located on the main floor of the Poplar Center, situated on 5504 Macleod Trail near Chinook Centre.
This location offers excellent access from all quadrants of the city.
There are more than 200 dedicated parking spots to ensure ease of access to our patients. We recommend our patients to park in the underground parking lot.
